Transfer of Records policy

 

Medical Records Access & Transfer.

Patients have the right to access their medical records under Australian privacy law. We’re happy to provide either a Health Summary or a copy of your full medical record, and we will manage all requests securely and respectfully.

What you can request

  • Health Summary (e.g., diagnoses, medications, allergies, key history)

  • Full record copy (e.g., consultation notes, results, letters, care plans)

How to request

Please submit your request in writing (email or in person) and include:

  • Your full name and date of birth

  • What you’re requesting (Health Summary or Full Record)

  • How you want it provided (secure email, post, collection, or sent to another clinic)

We may ask for photo ID before releasing records.

Timeframes

We aim to process requests within 30 days.

Fees

  • Health Summaries are usually free.

  • A reasonable administrative fee may apply for full record copies to cover costs such as staff time to prepare the file, printing, or postage.

  • We will confirm any fee with you before processing. Payment may be required before release.

Transfers to another clinic

If you are moving to a new GP, we can send a Health Summary (usually free). Full record copies may attract a reasonable admin fee.

We provide records using secure methods and protect your privacy at all times.
